TERMS AND CONDITIONS
AUTHORISATION FOR MEDICAL SERVICES
In the event of illness or injury, a reasonable effort will be made to contact you to obtain consent in advance of medical services being given. If we are unable to contact you, the Director/Principal will consent to such services for your child by acting on your behalf on your advance authorisation below:
I have read the above and hereby designate the Director/Principal or my designated emergency contact person to act on my behalf in the event of an emergency. He/She may authorise medical attention and the most appropriate medical facility as may be required in an emergency because of illness or injury sustained. I hereby assume all financial responsibilities for any medical condition or treatment that might be required and acknowledge that ISPP shall not be liable for any injuries incurred. I certify that all information given on the health history form is complete and correct, to the best of my knowledge. I will be responsible to update my child’s health history with the school as necessary and upon request.
a) we have read and understood the schedule of all fees and additional charges due to ISPP on enrolment of our child to ISPP;
b) we will pay all fees and additional charges due to ISPP for our enrolled child and understand that non-payment of fees on or before the required dates may lead to exclusion of our child from ISPP;
c) we are jointly (together) and severally (alone) liable for payment of fees and additional charges due, irrespective of any commitment from a company or organisation to make full or partial settlement of the charges; and
d) we will inform the Finance Office directly of any changes in billing details.
ENGLISH AS AN ADDITIONAL LANGUAGE (EAL)
ISPP staff may determine that a student requires English as an additional language even though an alternative language course may be preferred. I understand that if my child is assessed as requiring EAL assistance that it is a compulsory programme.
PARENT CODE OF CONDUCT
A positive, collaborative, working relationship between the School and a student’s parents/guardians is essential to the fulfillment of the School’s mission. One of the parameters of that mission is that the School does not tolerate behavior that is detrimental to the safety, integrity, or dignity of any individual or group. Therefore, the School reserves the right to discontinue enrolment if it concludes that the actions of a parent/guardian make such a relationship impossible or otherwise seriously interfere with the School’s accomplishment of its educational purposes. All nominated guardians will be required to agree to and sign the ISPP parent code of conduct at the time of enrolment.
I understand my child/ren may be transported by a school-operated vehicle for school activities during the school day.
USE OF YOUR CHILD’S PHOTOGRAPH IN SCHOOL PUBLICATIONS
From time to time, photographs are taken at school and during school events featuring ISPP students engaged in various school activities. These photographs are also considered for inclusion in various school publications as well as official social media platforms.
If, for any reason, you prefer that any photograph of your child NOT be used in any of the school’s publications, please send a written note informing the child’s Principal of your wishes. The exception to this is the school’s annual yearbook which contains photographs of every student in the school and where the distribution is controlled by the school.
If we do not receive a written request from you to exclude photographs of your child in other school publications, we will assume that you are agreeable to the use of such photographs which include your child.
